Principle Responsibilities & Position Purpose: The Outcomes Manager under the direction of the Director of Employment Programs is responsible for the supervision of the case managers, outcomes coordinators and community coaches and will oversee consumer issues within the WAP, SEP, IECBS and Discovery Programs. The position is responsible for all phases of consumer services for a specified case load including Individual Habilitation Service Plans, consumer contact notes, case conferences, discharge summaries, etc.; is also responsible for the coordination of services provided in all Employment programs including consumer programming, issues, rights, advocacy, and volunteer opportunities in order to be in compliance with all regulations including WIOA and HCBS. ESSENTIAL FUNCTIONS (This list is not all inclusive. A full description will be provided if and when candidate is selected for an interview.) • Acts as liaison with community, representing the agency in a professional manner. • Oversees the rehabilitation orientation/training for program participants following the Anthesis program description and agency policies. • Observes, records, evaluates, designs, implements, and monitors Individual Habilitation Service Plans for each consumer on assigned case load. Instructs and supervises the appropriate personnel to assist in implementation of the Individual Habilitation Service Plans. • In conjunction with other staff in program, holds case conferences at specified times with consumers, Anthesis personnel, parents, care providers, Regional Center staff, Department of Rehabilitation, and others. As an advocate, represents the needs of consumers to Anthesis personnel, parents, care providers, Regional Center staff, Department of Rehabilitation, and others. • Provides counseling, assistance in vocational training and other case management supportive services to consumers on assigned case load. Coordinates vocational training activities under the guidance of the Director. • Works in collaboration with the Outcomes Coordinator and Community Coaches to perform job duties in determining appropriate, integrated, community-based activities and appropriate planning for each participant during program hours. May fill in as direct support staff to meet the needs of participants. • Develops training supports for parents and care providers. Develops community, business, recreational and educational IECBS community activities in collaboration with the Outcomes Coordinator and individuals served. • Maintains relationships and follow-up with all community liaisons on an on-going basis to insure participant success. • Partners with the Outcomes Coordinator, individuals and their circle of support to evaluate participant progress on an annual and semi-annual basis. May serve in an instructional capacity by conducting trainings related to work, safety, and advocacy in the event of needed activities in the absence of paid work. Completes necessary reports as needed. Serves as a member of the Intake/Eligibility Committee. • Assures that consumer rights and confidentiality are respected. Responsible for quality of case records in accordance with systems and procedures of the agency, and standards of accreditation. Assures personnel in responsible programs are trained in person-centered planning. • Participates in sales presentations for local business, school fairs and volunteer groups, etc. This function may intersect with efforts of management overseeing Day Programs. • Ensures compliance with the standards of licensing, certification, and Federal/State Departments of Labor, accreditation agency. Ensures that all organization activities and operations are carried out in compliance with local, state and federal regulations and laws governing business operations. SUPERVISORY RESPONSIBILITIES: Manages subordinate employees who act as case managers, coordinators and/or community coaches in the Employment program; is responsible for the overall coordination and evaluation of the Employment programs. Carries out supervisory responsibilities in accordance with the organization’s policies, procedures and applicable laws; responsibilities will include but are not limited to, interviewing, hiring, directing work, appraising performance, rewarding and disciplining subordinate positions, addressing complaints and resolving problems, all within Employment programs.
SPECIFIC JOB KNOWLEDGE, SKILL AND ABILITY: The individual must possess the following knowledge, skills and abilities and be able to explain and demonstrate that he/she can perform the essential functions of the job, with or without reasonable accommodation. Demonstrated ability to work well with persons with disabilities, elderly persons, families, representatives of residential facilities, and referring agency staff; ability to speak and communicate effectively with consumers, staff members, care providers, family members, community members, etc.; a second language is desired but not necessary; ability to effectively deal with internal and external customers, some of whom will require high levels of patience, tact, diplomacy to defuse anger, collect accurate information and resolve conflicts. EDUCATION/EXPERIENCE: Must possess high school diploma or equivalent; Bachelor’s Degree from an accredited college or university in human services related field OR any combination of at least 3 years’ experience in human services delivery system, education, and training that demonstrates the ability to perform the duties of the position. Must have basic knowledge of and experience in, use of internet and email. Proficient in MS Office 2010 or later, particularly in Word, Excel, and PowerPoint. Database experience and Publisher a plus. Minimum of 5 year(s) experience in the field of rehabilitation social services or disability service system is desired. Employment history of increasing administrative responsibility in rehabilitation, social services or disability services system. Requires management experience and strong supervisory skills. Proficient in MS Office 2008 or later, particularly in Word, Excel, and PowerPoint. Basic level use of internet and email. Database experience a plus.
