Our mission is “Building community one family at a time, through compassion, advocacy and comprehensive model services, fostering self-sufficiency and sustainable impacts.” Position Overview The Licensed Vocational Nurse (LVN) is responsible for rendering nursing care and health education to individual patients and larger groups of the community in an assigned community based program under the direction of the program manager. General Responsibilities • Obtain proper consent from patients prior to rendering services. Health Education and coordination of care with primary care physicians and specialists. • Provide services based on individualized client needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of vocational nursing • Perform basic health screenings on assigned patient populations (blood pressure, blood glucose, temperature, heart rate). • Maintains complete, confidential and secured ongoing record of health screenings/vitals on selected patients who are receiving ongoing monitoring. • Provide ongoing case support to patients experiencing chronic disease where ongoing screening of basic vitals will assist in long term health outcomes. • Provide education and encouragement to patients to avoid unhealthy routines and habits that are negatively impacting their health and chronic disease processes. • Provides general health education to large population of community members targeted towards disease trends and issues as seen in the local health data as affecting population (s). • Develop and implement health education curriculum both for group education and individual patient education as needed. • Accompany patient(s) to doctor’s appointments as requested by patient, to assist with understanding of medical issues and to better coordinate care with physicians. • Assists patients with pharmacy related issues and concerns as requested by patient. • Provides resources and referrals for ancillary services as needed by patients. • Research and provides assistance for patients in obtaining resources (insurance, Medicaid, foundation coverage) for medical assistive devices as needed (such as canes, walkers, raised toilet seats, hand rails, adult urinary care supplies, diabetes testing supplies, etc.)
 
Qualifications • Licensed Vocational Nurse (LVN) • Must have knowledge of normal glucose, cholesterol and blood pressure levels. • Current BLS mandatory. • Phlebotomy experience and knowledge of needle disposal. • California Driver’s License. • Reliable automobile. • Ability to interact with public and relay resources as needed. • Excellent follow-up skills • Background and Fingerprint Clearance • Current TB Clearance • Completion of formal or informal education sufficient to assure the ability to read and write English and to communicate at the level required for successful job performance.
