Anthesis, formerly known as PVW, is a nonprofit organization in Montclair, CA seeking to fill a Job Coach position created with the intent of carrying out Anthesis goal and mission of assisting adults with intellectual disabilities live and work to their fullest potential. As a Job Coach, you will have the opportunity to instruct adults with intellectual and developmental disabilities in activities designed to promote vocational, social, physical, and intellectual growth. The goal will be to help these individuals succeed and become fully integrated into their surrounding communities. The position requires the ability to accurately document, record and communicate information on consumers including but not limited to: tracking progress, work performance, transportation issues and community employer information which will include coordinating consumer work schedules with community employers. The job coach will also be responsible for maintaining a supportive relationship with their consumers by employing careful job training and correct job placement/assignments to enhance or improve consumer integration into their workplace and/or communities. The consumer/job coach relationship will be similar to that of a boss/subordinate in the sense that as a job coach, the employee will be held accountable for tracking consumer productivity, behavior changes, punch in/punch out (payroll) and any other off-site consumer employment activities. Other case management activities will be assigned as needed. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other Duties may be assigned. Is responsible to ensure all job duties are performed in-line with and in support of the mission of Anthesis. 2. Responsible for accurate documentation of production records, including piece rates, input data into Intuition and any other information relating to employee pay or program service billing. 3. Attends required staff meetings and assists in discussion of off-site activities. 4. Responsible for the work performance of consumer workers assigned to the community employer location. 5. Responsible for maintaining a folder with consumer worker information at all times. 6. Responsible for maintaining a transportation log when applicable. 7. Responsible for monthly consumer worker meetings and documentation of safety and rules of conduct of community employer location. 8. Responsible for any documentation related to off-site billing for work performed. 9. Coordinates production scheduling through written and/or oral instructions. 10. Maintains a supportive relationship, careful training, and correct job placement/assignments to enhance or improve consumer worker capabilities. 11. Sets a good example of rules and directives of community employer, displaying to consumer workers the proper techniques, habits, and attitudes for work. 12. Makes recommendations to program manager for any improvement to simplify or increase production. 13. Maintains quality production through continual inspection of consumers' work. 14. Maintains accurate, individualized accounting of consumer workers' attendance, working hours and production. 15. Sets up proper workstations and layouts when required. Simplifies work methods when possible and assists consumer workers to meet production schedules. 16. Determines needed materials, parts, tools, & equipment to complete jobs &keeps consumer workers supplied w/such. 17. Analyzes and corrects production problems and slow-downs. Controls waste of material and time. Deals effectively with problems which hinder employment such as defiant behavior, hygiene, grooming, low productivity. 18. Deals with consumer worker grievances or complaints in conjunction with program manager. 19. Assists in the compilation of data for the program evaluation system. 20. Responsible for accurate work performance records utilizing Case Contact Notes. 21. Aids program manager and case manager in developing and implementing Individual Habilitation Plan for each consumer. 22. Responsible for intervention in behavior management problems in compliance with Individual Habilitation Plans. 23. Responsible for charting consumer progress in consumer files. 24. Responsible for complying with the safety standards of the agency. 25. Responsible for the prompt reporting of injuries and other serious incidents involving the consumer to the program manager and case manager. 26. Responsible for safe operation of agency vehicles and for prompt reporting of all accidents and/or vehicle code to the Director of Work Services. Responsible for maintaining clean environment in agency vehicles. 27. Must maintain valid first aid and CPR certification. 28. Must possess valid California Driver’s License, 3-5 years of driving experience with an insurable driving record, current vehicle registration and proof of vehicle insurance.
EDUCATION and/or EXPERIENCE: High school diploma or general education degree (GED); Associate of Arts Degree in a related field desired; Sufficient experience with adults with intellectual and developmental disabilities to carry out duties required in above description; Must possess valid California Driver's License and three to five (3-5) years driving experience with a good driving record. Must possess proof of current vehicle insurance and proof of current vehicle registration. May be required to transport consumers in personal vehicle to and from work locations/facilities.
